In consideration of being permitted to use the simulator equipment, facilities, services, and amenities at Driven Interactive Sports Lounge, I hereby agree to the following terms:
1. ASSUMPTION OF RISK
I acknowledge and fully understand that participating in simulator-based sports (including golf, baseball, football, soccer, racing, and other virtual activities) involves inherent risks of personal injury, illness, or property damage. I voluntarily assume all such risks, known and unknown, and agree to participate at my own risk.
2. RELEASE OF LIABILITY
I, for myself and on behalf of my heirs, executors, administrators, and assigns, hereby waive, release, discharge, and hold harmless Driven Interactive Sports Lounge, its owners, agents, employees, affiliates, and successors from any and all claims, demands, causes of action, liabilities, or expenses (including attorney fees) arising out of or related to any injury, disability, death, or loss of personal property that may occur while on the premises or participating in any activity.
3. EQUIPMENT DAMAGE
I understand I am responsible for using all simulator and lounge equipment properly and respectfully. I agree that if I intentionally or negligently damage any equipment or property, I will be held financially responsible for repair or replacement.
4. ALCOHOL CONSUMPTION & PERSONAL CONDUCT
If consuming alcohol on-site, I agree to do so responsibly. I acknowledge that staff reserves the right to remove or refuse service to anyone exhibiting unsafe or disruptive behavior.
5. USE OF PERSONAL EQUIPMENT
If I choose to bring my own golf clubs or other personal gear, I understand and agree that Driven Interactive Sports Lounge is not responsible for any loss, theft, or damage to personal items.
7. MINORS
Participants under 18 must have this waiver signed by a parent or legal guardian. Children under the age of 16 must be supervised by an adult.
8. ACKNOWLEDGMENT
I certify that I am in good physical condition to participate, that I have read this waiver thoroughly, and that I understand its contents. I agree that this waiver is binding to the fullest extent permitted by Georgia law.
